MOH Reports 2‑Hour Wait at NUH Not Linked to Woman’s Miscarriage

MOH Reports 2‑Hour Wait at NUH Not Linked to Woman’s Miscarriage

What Really Happened: A Hospital Misstep

In a nutshell: The Ministry of Health (MOH) has wrapped up its review of the National University Hospital’s (NUH) investigation into a painful miscarriage case. The hospital did a deep dive – watching videos, talking to staff – and found faults but not the fault that caused the loss. Yet, certain gaps need patching.

Behind the Investigation

  • Senior doctors and nurses from emergency, obstetrics & gynaecology (O&G) and the clinical governance team led the probe.
  • Video footage and staff interviews formed the backbone of the inquiry.
  • It revealed that while existing protocols were sound, the flow of patients on that night hiccuped.

The Gate‑In Failure

NUH had a speedy “rapid access” route: If a pregnant patient showed signs of labor, she would be whisked straight to the delivery suite and seen by an O&G doctor. No detours through the ED doctor first.

But that night, the ED was flooded, the delivery suite was full, and the smooth transfer simply fell apart. The patient didn’t get the timely care she deserved.

New Safeguards

  • Triage Boost: Pending transfers will get an extra check by the ED nurse and a quick review by the ED doctor to touch base with the O&G specialist.
  • Ultrasound On‑the‑Spot: Pregnant patients can be scouted with ultrasound right in the ED if needed.
  • Standard Score: A uniform obstetric assessment score will now help triage these patients efficiently.
  • Heart‑Beat Expectation: After O&G review, fetal monitoring will kick in while the patient waits for a delivery suite bed.

Heartfelt Apology

NUH sat down with the patient and her husband, explained what went wrong, and clarified the miscarriage likely didn’t happen in the hospital. The couple accepted the explanation and recognized that life’s twists aren’t always within a hospital’s hands.

NUH assured them that the hospital is tightening processes, so such mishaps stay in the past.

Keeping Everyone in the Loop

MOH has shared NUH’s findings with senior management across all public hospitals, ensuring the learnings echo far beyond just NUH.
