Unsterilized Dental Instruments at TTSH Spark Patient Safety Alarm

What Actually Happened at Tan Tock Seng Hospital

After a routine check, staff discovered that a dental instruments had skipped the final sterilisation step. The hospital later found eight packs that were used on patients without being fully sterilised.

Who Might Have Been Affected?

  • Up to 8 out of 575 patients who visited between Nov 28 and Dec 5.
  • Staff are now contacting each patient to offer support and answer any questions.

Emergency Halt on Elective Procedures

Since Saturday, the clinic has suspended all elective (non‑emergency) procedures to create a “safety timeout.”

Procedures are scheduled to reopen tomorrow.

How the Sterilisation Process Should Work

Normally, the process at TTSH follows three steps:

  1. Hand wash, ultrasonic treatment, and an enzymatic solution.
  2. Air dry, pack, seal, and label.
  3. Steam sterilisation—this is where the slip happened.

TTSH says the first two steps already kill most “organisms of concern,” so the risk was expected to be “extremely low.” Yet the oversight slipped through.

What the Minister is Saying

Health Minister Gan Kim Yong expressed disappointment at the lapse, stating:

  • “We take a serious view of this incident…”
  • “MOH will study it closely, consult experts, and explore more safeguards to prevent a repeat.”

Next Steps for the Ministry

  • Direct the National Healthcare Group and TTSH to carry out thorough reviews.
  • Require a formal report detailing findings and actions to the ministry.

Why This Matters

Even a small oversight in sterilisation can lead to serious patient concerns, so the government’s comprehensive response is aimed at reinforcing trust and safety across the healthcare sector.
